There would be much disparity (and many more countries and cultures) in the Middle East and Asia. Alexander united the countries he conquered, spreading one major way of life while still allowing all the different cultures to co-exists (excluding the Zoroastrians...)

I imagine Persia would've been the main country and culture, since that was the biggest power until Alexander took the Macedonians into the stratosphere. Persian customs and culture would've severely hindered and/or replaced the largely Roman culture that influenced our culture today.
